sendmail: General

- I/O activity: abstract out the use of select(2)
to enable selection of other methods, e.g.,
poll(2), poll(7d) (Solaris),
kqueue(2) (*BSD).
- Threading: instead of creating one thread for each connection,
use a worker model and allow for a certain number of threads
that are used to multiplex the work.
This should enhance the performance on several OSs.
